MASS INTENTIONS AND SACRAMENTAL RECORDS
Mass Intentions
In our community, we understand the importance of offering intentions during Mass to express our petitions, thanks, and special remembrances. If you wish to request an intention to be included in our Masses, whether during the weekend or any day of the week, please follow these steps:

Call our parish office at 773-280-7700 to request your intention.

You can request intentions for various reasons, such as health, birthdays, or in memory of someone who has passed away, among others.

A donation of $20 is requested for each intention. This contribution helps our parish continue its mission and maintain our activities.

To ensure that we can include your intention in the desired Mass, please make your request at least one week in advance. For example, if you want your intention considered for a Mass next Monday, you should contact us no later than the previous Saturday.
Sacramental Records
For those in need of sacramental records such as baptism, first communion, or confirmation, we provide an online process to assist you. Whether you are requesting a certificate from Saint Mary of the Lake Parish, or Our Lady of Lourdes Parish or our combined parish, follow these steps to obtain your document:
Contact: Complete the online form to request your certificate. Or contact the parish office.
Information Required: Please provide the specific type of certificate you are requesting (baptism, first communion, confirmation, heritage), specify the church (Saint Mary of the Lake or Our Lady of Lourdes) and provide your name, date of birth, parents’ names, and a contact number. You will also be required to supply a government-issued I.D. This information will enable us to locate your document effectively.
Donation: A donation of $20 is requested for each certificate. This contribution supports the maintenance and operations of our parish. If you are experiencing financial hardship, please contact the parish office.
Processing Time: The processing time for certificate requests is approximately 10 to 14 days. Sacramental records are not electronic and must be manually located in the parish archives.
Notification: Once your certificate is ready, our parish office will contact you to notify you. We will mail the certificate to you or you are welcome to pick it up from the parish office.
